- Be a Carroll County resident
- Be uninsured
- Have a household income that is less than 200 percent of the federal poverty guidelines

Access Carroll sees patients by appointment only. Patients should call 410-871-1478 to schedule an appointment.
What to bring to your first appointment:
- Driver’s license or photo ID and a birth certificate
- Proof of Carroll County residency (e.g. lease, current utility bill, letter from your landlord or shelter)
- Your last four paycheck stubs
- All documentation from the State of Maryland or the Social Security Administration for any assistance you receive including, child support, alimony, disability, housing and food stamps
- Your most recent Federal Income Tax Return and W-2 form
- Asset information including current bank statements
