Services Provided by Access Carroll

Access Carroll provides the following services:

  • Primary health care
  • Medication assistance
  • Laboratory testing
  • Radiology studies
  • Referrals to specialists
  • Patient education
  • Information about community resources

To receive care from Access Carroll, you must:

  • Be a Carroll County resident
  • Be uninsured
  • Have a household income that is less than 200 percent of the federal poverty guidelines

Appointments

Access Carroll sees patients by appointment only. Patients should call 410-871-1478 to schedule an appointment.

What to bring to your first appointment:

  • Driver’s license or photo ID
  • Proof of Carroll County residency (e.g. lease, current utility bill, letter from your landlord or shelter)
  • Your last four paycheck stubs
  • All documentation from the State of Maryland or the Social Security Administration for any assistance you receive including, child support, alimony, disability, housing and food stamps
  • Your most recent Federal Income Tax Return and W-2 form
